Terms and Conditions for Purchasing a Whitening Package Gift from Space Dental
- Eligibility:
The Whitening Package is sold with the understanding that the recipient (the patient) will attend an initial consultation and hygiene appointment at Space Dental before any whitening trays or gels are provided. - Consultation and Hygiene Requirements:
- The patient must undergo a thorough dental examination to assess their oral health and suitability for whitening treatment.
- A hygiene appointment may also be necessary to ensure the teeth are in optimal condition for whitening. These preparatory appointments are mandatory and are not included in the cost of the whitening package unless explicitly stated.
- Suitability for Treatment:
- If the patient is deemed unsuitable for whitening treatment due to oral health concerns (e.g., cavities, gum disease, or other issues), these concerns must be addressed before the whitening treatment can proceed.
- The cost of achieving oral fitness (e.g., fillings, periodontal treatment) is the responsibility of the patient and is not included in the whitening package.
- Explanation of Treatment Plan:
- During the consultation, the dental team will explain any necessary treatments required to achieve oral fitness, including associated costs and timelines.
- The patient has the option to proceed with recommended treatments at their discretion and expense, either with Space Dental or another dental provider of their choice.
- Non-Refundable Nature of the Package:
- The Whitening Package is non-refundable. By purchasing this package, the buyer acknowledges that additional costs for oral fitness may be incurred by the recipient if required.
- If the patient chooses not to proceed with the necessary preparatory treatments, the whitening treatment will not be provided, and no refund will be issued.
- Expiration of Package:
- The Whitening Package must be redeemed within 12 months of the purchase date.
- Appointments are subject to availability, and it is recommended to book early to ensure timely completion of the whitening process.
- Consent to Treatment:
- The recipient must sign a consent form acknowledging their understanding of the procedure, potential risks, and the need for oral health readiness before whitening treatment can commence.
- Transferability:
- The Whitening Package is transferable to another individual if the original recipient does not wish to proceed, provided the new recipient agrees to the same terms and conditions.
By purchasing the Whitening Package, you confirm your understanding and acceptance of these terms and conditions. Space Dental reserves the right to amend these terms at any time without prior notice.
